Digital health ID card online registration process
Your digital health ID card can be generated using your Aadhaar, mobile number, or driving licence.
i) Using Aadhaar
Following are the steps for applying online for an NDHM health card using your Aadhaar number.
Step 1: Go to the NDHM website and click the “Generate ID” button.
Step 2: Choose “Generate via Aadhaar,” enter your Aadhaar number on the new page, and click the submit button.
Step 3: An OTP is sent to your registered mobile number, which must be entered in the required field.
Step 4: Create your digital ID by entering your personal information and selecting a username and password.
Step 5: Log in again with your newly created username and password, and enter your address.
Step 6: Download your digital health ID card for future use.
- ii) Using a Mobile Number
If you do not wish to use your Aadhaar or DL to generate your health ID card, you can generate a digital health ID using your mobile number.
Step 1: Go to the NDHM website and click the “Generate ID” button.
Step 2: If you do not have any IDs or do not want to use IDs, you can generate them. Select the “Click here” button.
Step 3: Enter your mobile number to generate an OTP, and then enter the OTP in the appropriate field.
Step 4: To create your digital health ID, enter your personal information and select a username and password.
Step 5: Log in with your newly created username and password and enter your address.
Step 6: Download your digital health ID card.
iii) Using a Driving License
The following are the steps to apply online for an NDHM health card 2021 via your driving licence (DL).
Step 1: Go to the NDHM website and click the “Generate ID” button.
Step 2: Click the “Generate via Driving License” button. A popup window informs you that after you submit your information, you will be assigned an enrolment number.
Step 3: Then bring your enrollment number to a nearby participating facility to obtain your digital health ID.
Eligibility Criteria for Digital Health Card
Anyone who wants to take part in the Ayushman Bharat Digital Mission can apply for a health ID. There are no specific requirements for generating a health ID card.

How to deactivate your health ID card?
The health ID card is optional, and you can choose to move out of the programme and reactivate it whenever you require it again. Here’s how to deactivate/delete your digital health ID.
Step 1: Enter your health ID or PHR and date of birth, then click submit.
Step 2: To log in to your account, you must generate an OTP using your Aadhaar or mobile number or use the password you set.
Step 3: After logging in, go to “My Account” and select “Deactivate/Delete Health ID” (deactivation of health ID is temporary, and your account will not be closed; however, you can delete the health ID) and then tap “Continue” to delete or deactivate your health ID.
